With DocResponse, you’ll be able to complete all required forms and enter your medical history prior to your visit, wherever &whenever you want!
3 days before your appointment, you’ll begin to receive text & email notifications asking you to complete your pre-visit registration from DocResponse.
The data you enter is securely transferred to your chart for review by your provider, so they’ll have it at the time of your arrival.
All patients need to arrive at least 10 minutes prior to their scheduled appointment to complete the check-in process.
No Show Policy
Appointments
As of 2025 No Show Fees for appointments that are not cancelled more than 24 hours prior will be as follows:
- Established patient – $50
- New patient – $100 (Must be paid prior to rescheduling)
- Ultrasound patient – $50
Surgeries
- Surgeries must be cancelled 48 hours prior to the day of a surgery procedure.
- $150 will be charged for Surgery No Shows.
